Position Summary:
The Homeless Outreach & Engagement Program Manager is responsible for leading, supervising, and developing outreach strategies to identify, engage, and connect individuals experiencing homelessness to services, housing, and support. Key Responsibilities:
Program Oversight & Team Supervision
- Lead, supervise, and support outreach staff and volunteers across designated service areas.
- Ensure consistent implementation of outreach protocols and service delivery standards.
- Provide staff training, performance evaluations, and professional development.
Outreach Strategy & Community Engagement
- Design and coordinate outreach activities in encampments, shelters, and public spaces.
- Build relationships with individuals experiencing homelessness to foster trust and engagement.
- Represent CHASS in outreach collaborations, community events, and coalition meetings.
Service Coordination & Navigation
- Oversee assessment, intake, and referral processes, including connection to housing, healthcare, and mental health services.
- Ensure follow-up and care coordination with case managers and partner agencies.
- Track participant outcomes and ensure continuity of care across services.
Program Evaluation & Reporting
- Monitor program goals, data collection, and performance metrics using HMIS and internal systems.
- Prepare and submit regular reports to funders and organizational leadership.
- Identify gaps in services and recommend program improvements based on outreach trends.
Risk Management & Safety
- Ensure safety protocols and crisis intervention procedures are followed by all team members.
- Maintain communication and coordination with law enforcement, EMS, and emergency response providers as needed.
Qualifications:
- Minimum 4–6 years of experience in homeless outreach or community-based services, including 2+ years in a supervisory role.
- Strong knowledge of homelessness issues, harm reduction, trauma-informed care, and local resources.
- Demonstrated leadership, communication, and crisis management skills.
- Proficiency in HMIS and case management systems.
- Valid driver’s license and ability to conduct fieldwork required.

Position Summary:
The Outreach Case Worker/Support Staff plays a vital role in engaging individuals experiencing homelessness and connecting them to services and resources. This position conducts street-based outreach, distributes supplies, builds relationships with unsheltered individuals, and supports the broader outreach team in assessing needs, making referrals, and facilitating access to shelter, healthcare, and housing. The role requires compassion, reliability, and a commitment to reducing barriers for vulnerable populations.
Key Responsibilities:
Outreach & Engagement
- Conduct street outreach in targeted areas, including encampments, parks, and public spaces.
- Build trust and rapport with individuals experiencing homelessness.
- Distribute hygiene supplies, food, clothing, and resource information.
Service Connection & Support
- Assist with referrals to shelters, day centers, and emergency services.
- Support clients in completing intake forms and navigating access to basic services.
Collaboration & Communication
- Work closely with case managers, housing navigators, and outreach teams to coordinate services.
- Communicate observed client needs and changes in condition to the appropriate staff.
- Participate in staff meetings, training, and case conferencing.
Documentation & Reporting
- Maintain accurate records of outreach activities and client interactions.
- Enter data into tracking systems or logs as required.
- Assist with compiling reports and meeting program deliverables.
Qualifications:
- Prior experience working with individuals experiencing homelessness or in community-based settings is preferred.
- Strong interpersonal and communication skills.
- Compassionate, nonjudgmental, and culturally sensitive.
- Ability to work independently in the field and in varying weather conditions.
- Must have a valid driver’s license and be comfortable navigating urban areas.
